The Giessen district is one of the 73 "Smart Cities Model Projects" (MPSC) that have been funded by the federal government since 2019 and develop and test cross-sector, sustainable digital strategies. The subject of the present procedure is the procurement, introduction and operation of a web-based, cross-sector digital aftercare platform including a mobile app for the structured care of geriatric patients and patients in need of care after hospital discharge, starting with a pilot for initially around 50 patients at the Asklepios Clinic in Lich in the Giessen district. The aim is to reduce avoidable readmissions as well as prioritized, indicator-supported follow-up care through continuous, automated monitoring Patient categorization in the traffic light system (green/yellow/red) and a virtual care team that enables telemedical monitoring and intervention. The "Licher Model - Digital Aftercare" project is funded in the "Strong Home Hesse" program and addresses, among other things: demographic and Structural care challenges in rural areas by closely networking clinics, general practitioners and specialists, nursing staff, relatives and volunteers.
