- The subject of the contract according to § 140a SGB V is a telemonational monitoring of insured persons distributed to three lots in the indications of heart failure, COPD and diabetes mellitus type 2. Possibility opens up to reflect and optimize your own health behavior. Regular advice and training - including both medical and psychosocial aspects - should strengthen the competence of the patients in dealing with their illness. This helps to increase your quality of life, since you can not only better inform and motivate, but also to influence the course of the disease. In addition, hospital admissions should be avoided. The Performance is divided into three looses to the following indications: LOOK 1: Heart failure LOS 2: COPD Los 3: Diabetes mellitus type 2 per lot, the TK assumes that 1,000 pension cases per contract year assume. The maximum quantity is 2,000 registered pension cases per Contract year per lot. The contract is concluded for a permanent time and can be terminated with a period of six weeks at the end of the quarter.
